Questionnaire Sum Assessment Builder
The Questionnaire Sum Assessment Builder combines multiple questionnaire responses into a single score by summing the values of selected questions.
This assessment is commonly used for wellness questionnaires, readiness surveys, mental health screenings, recovery assessments, fatigue monitoring, and other multi-question forms where an overall score is more meaningful than reviewing each question individually.
By aggregating multiple responses into a single value, coaches and practitioners can quickly identify trends, monitor athlete status, and apply thresholds to generate alerts or readiness indicators.
Assessment Name
Enter a unique name for the assessment.
This name is used internally when editing, assigning, or managing layouts.
Examples:
- Daily Wellness Score
- Recovery Questionnaire Total
- Readiness Index
- Fatigue Assessment
Display Name
The column header displayed within dashboards and reports.
This can be a shortened version of the assessment name.

- Wellness
- Recovery
- Readiness

Questions
Select the questionnaire fields that should be included in the total score.
Each selected question contributes its recorded value to the final assessment result.
Examples:
A wellness questionnaire may include:
- How restful was your sleep last night?
- How well did you hydrate yesterday?
- How difficult was yesterday's training session?
- How sore do you feel today?
- How stressed do you feel today?
The Questionnaire Sum Assessment will add all selected responses together to create a single score.
Question Order
Questions can be rearranged using the drag-and-drop controls.
This allows administrators to:
- Organize questionnaires logically
- Group related questions together
- Maintain consistency across assessments
Changing the order does not affect scoring.
Adding Questions
Use the Add (+) button to include additional questionnaire fields.
There is no requirement that all questions come from the same questionnaire, provided they use compatible scoring scales.
Removing Questions
Questions can be removed using the delete icon beside each selected field.
Removing a question immediately excludes it from the calculated score.
Show Value
Determines whether the calculated score is displayed.
Enabled
Displays the total questionnaire score.
Example:

Disabled
Displays only the assessment icon or threshold indicator.
This is useful when the score itself is less important than whether it falls within an acceptable range.
Use Latest Value
Determines how questionnaire responses are selected.
Disabled
Only responses submitted on the selected dashboard date are considered.
Enabled
The most recent completed questionnaire is used, even if it was submitted before the selected date.
This setting is commonly used when questionnaires are not completed every day.
Icon
Allows an icon to be displayed alongside the questionnaire score.
Icons can be used to quickly communicate:
- Readiness status
- Recovery status
- Compliance
- Alert conditions
- Wellness categories
Thresholds and Color Coding
Thresholds can be configured to automatically classify questionnaire scores.
This allows coaches and practitioners to quickly identify athletes who may require additional attention.
